Create your AZOVA account and upload a photo of your vaccine card and government issued photo ID. The record must include your first and last name, date of birth, administering organization and the names and dates of the vaccine(s) that you have received.
Note: If you are creating a record on behalf of a dependent, first create your own account and then add them as a dependent to create their record.
Your digital vaccine record will be confirmed and we will automatically create your COVID Credentials™ Simple Health Pass for you. You will be notified via SMS and email with a link to view your health pass.

Your record will be confirmed for a match between your demographic information, photo ID, vaccination card and self-reported vaccination information.
The AZOVA platform is HIPAA, SOC2, HITRUST, and GDPR compliant. We believe that you should be in control of your data no matter what circumstance the world is in. By default, we share only the minimum required information as requested by the organization you are sharing with and only with your consent.
