How it works

STEP 1: Create your electronic vaccination record and request confirmation from AZOVA.

Create your AZOVA account and upload a photo of your vaccine card and government issued photo ID. The record must include your first and last name, date of birth, administering organization and the names and dates of the vaccine(s) that you have received.

Note: If you are creating a record on behalf of a dependent, first create your own account and then add them as a dependent to create their record.

Step 2: Get your electronic Vaxigo vaccination record confirmed.

Your digital vaccine record will be confirmed and we will automatically create your COVID Credentials™ Simple Health Pass for you. You will be notified via SMS and email with a link to view your health pass.

STEP 3: Consent to share your vaccination status with Rakuten Americas.
We will automatically share your vaccine record with Rakuten Americas. You can use your health pass for work, travel, school and entertainment. Anytime you receive another COVID-19 vaccination, simply complete steps one through three again.
STEP 4: With your consent, your vaccination record status will be shared with Rakuten Americas automatically.
Your vaccination record status will automatically be shared when you consent for us to do so. We will also provide you with an electronic vaccination record, COVID Credentials and a free Health pass for travel. You can also share with anyone you’d like such as airlines, hotels, employers and schools at any time.

Note: If you already have an electronic Vaxigo vaccination record, please complete the registration here to consent to share your vaccination status with Rakuten Americas.
